Mandatory Annual Consent Forms

Mandatory Annual Consent Forms- Each student must complete all 5 forms

1. Short Local Excursions
(ex. Walking trails, Terry Fox Run, Walk for Wenjack, soccer fields, splash park etc.)

2. Early Dismissal
In the event of an emergency evacuation or inclement weather

3. Electronic Communication
School monthly calendars, Week at a Glance, school sports, fundraisers & events etc.

4. R elease Personal Information
Pictures, videos, art/science work within the school and/or on school social media.

5. Digital Technology Student User Agreement
Acknowledgment of the rules & responsibilities when using any school computers or technology (ie. Smart boards, Chromebooks).***This is required from all students within the school (K-8).
